I agree to provide the employer with any additional information for verification, and I will immediately notify Sentraal Suid Kooperasie if any information changes.

I hereby give Sentraal Suid Kooperasie permission to collect, verify, process, and store my personal information, as defined in the Protection of Personal Information Act (ACT 4 of 2013) and the Promotion of Access to Information Act (Act 2 of 2000).
